We securely store this enquiry so our team can review it and respond. Where email delivery is configured, the enquiry is also sent to info@doll-aesthetics-beauty.com. Please do not include medical history or other sensitive clinical information in this form.

Our on-site appointment request collects your name, email address, optional phone number, selected service, requested time, and whether you are enquiring about botulinum toxin injections. We use these details to manage the request, contact you, and arrange payment through Stripe Checkout. Stripe processes payment details under its own privacy policy; card details are not stored on this website. A request is provisional until the clinic confirms it after payment is verified.

Appointment emails may include clinic-approved blank consent PDFs. Do not email completed forms or medical history in reply. The clinic arranges consent and clinical information securely or in person. Voucher purchases and Academy course bookings may still use separate third-party services, which operate under their own privacy policies.

Medical & Clinical Data

We strictly separate website appointment requests from clinical data. We do not ask for sensitive medical history or completed medical forms on this public website. Clinical screening, prescribing decisions, medical history, and completed consent forms are managed securely through the clinic's approved process or in person.

Analytics

This website does not deploy invasive tracking. If we implement analytics in the future, it will be privacy-focused, requiring your explicit consent before activating, and will never share personally identifiable information.
